Biggest e-learning mistakes

What do you think the biggest e-learning mistakes are? So far I have in no particular order:- No instructional design- Too much in one course (information overload)- No activities- Too complex- Memory questions only, not applied to the job- No printable summary- Underestimating the effort- Designing without knowing at least one development tool - Designing w/o instructional design training - Failure to focus on the performance objectives- No objectives- Questions on material not covered in course
