We made custom buttons for all our courses and as Ben states, we use upper right navigation; however, when they get to the testing pages, the top right navigation is still there, but we added bottom right buttons reduction of stress on the test taker. They can use whichever is comfortable and gives them more concentration on the task at hand and not on how to navigate. We also created a menu button. When we have large courses that have sections within chapters, the introduction page of each chapter becomes a type of TOC for the section and each page in the section has the menu button to return to the TOC of the chapter. We also have a left side list (TOC) of chapter headings. Kind of a situation where you have to see it to fully grasp what I am saying, but hopefully, you got it.
