I can do this for you if you are willing to key in the chapter names. Using a TOC is very risky because its vertical spacing depends sooooo much on the browser. Much safer to use either separate text boxes or a list of the names in one box and check Render as image.I can do this for you if you are willing to key in the chapter names.
