Survey Questions to solicit feedback on course quality?

Hello folks,
Our corporate sales training initiative is going into beta now. In order to gauge the quality of our courses, I plan to utilize Survey Monkey with a number of questions (about max. 5 minutes worth).
Does anyone have any suggestions for good qualitative questions?
Here are a few thus far (I haven't put much thought into this yet):
Was the content concise and informative?
Was the audio clear and easy to understand?
Where there any questions that you had that remained unanswered? What were they?
Did you find anything redundant or unnecessary?
What components were effective in helping your understanding of the material:
- Interactive components Scale: Not enough ---- Just Right ---- Too many
- Videos Scale: Not enough ---- Just Right ---- Too many
- Graphics Scale: Not enough ---- Just Right ---- Too many
- Quiz Questions Scale: Not enough ---- Just Right ---- Too many
Where the quiz questions clear and fair?
Was the flow of the lessons intuitive and in a logical order?
Did we provide enough technical knowledge? Scale: Too little
Too much
Did we provide enough sales knowledge? Scale: Too little
Too much
Did you feel like you are better equipped to start selling our solutions?
How would you rate your overall satisfaction with this course:
