What constructive feedback do you seek?

I am working on a survey to measure the efficiency of our Lectora titles. I'm using a different software, but I thought that the topic of CBT feedback would be relevant. Do you have questionnaires to measure the efficiency and clarity of your CBTs?I created the questions based on another survey form (using a Likert scale of Agree through Disagree). I changed it to reflect the online self-paced training rather than the instructor-led training. I wouldn't mind getting feedback on this list and maybe some suggestions that I hadn't considered.Name of Learner (optional): Date training activity completed:Course code: The course introduction explained the learning objectives of the class/module. This course accomplished the stated objectives. Information presented was sufficiently detailed and accurate. Course material was easy to understand. The course was engaging and held my interest. The course content was relevant to my job responsibilities and will assist me in performing my job more effectively. The course layout was conducive to the learning experience (no glaring grammatical errors, information laid out in easy-to-read chunks, etc.).Overall, I was satisfied with this class/module. Kevin
