What constructive feedback do you seek?

One problem with surveys is that you may not ask the critical question because you don't know what it is. A good way to be sure is to ask a lot of open ended questions of a small sample. Then formulate closed questions like you have to see how prevalent they are. So if you can, provide a way for them to give open feedback like: Other comments or suggestions:...Consider Level 2 and 3 evaluations.Level 2 is knowledge retention a day or so later - not right after the course.Level 3 is somehow (observation, interview, within a week to see if the training actually changed behavior - did they change the way they worked based on the training content.
