Test Questions Not Always Retaining Correct Answers

Hi all,
Sometimes when I insert a test question, once I hit OK to finish adding the question, the answer I selected as being correct is either unmarked (occurs in multiple response) or has reverted to the first choice (occurs in multiple choice).
Has anyone else experienced this? It is getting annoying.
