Test results randomly not recorded in LMS

I'm posting this question as hope that someone has some ideas for me, but I'm not expecting much. There are potentially so many variables involved in troubleshooting this problem. We've had our Learn.com LMS for 18 months now and have been using Lectora since then. For nearly that long I've been getting reports from a small percentage of users that they have completed a lectora course, i.e. they passed the test and viewed the "You Passed" page. The "completed" variable should have passed to the LMS upon viewing that page. For whatever reason the completed variable didn't pass to the LMS so they don't show as having completed the test. To fix it and show them as completed, my only recourse is to have them retake the test. When the test is passed again, the course marks as completed in the LMS, although sometimes it requires the user logging on to a different computer and taking the test there. Are there other LMS users who have experienced this same seemingly random problem? I've never been able to replicate the problem on demand, so I've always chalked it up to internet complexity in that we're a hosted client, SCORM relies on internet communication, our 50 sites are spread out over Montana and Wyoming where internet connectivity is sometimes not so great, and the connection must have dropped for just long enough to stop SCORM communication. Am I right or are there settings in Lectora or the LMS that could be affecting completion? When I publish to SCORM I use JavaScript Title Manager. Might that be causing a problem with some users? Our 1700 employees all run Win XP with IE 6 or 7. I haven't noticed any correlation between browser version and problems. In a typical course we might have 1500 completions and get reports of 25 people who say they completed it but it isn't marked as completed. Thanks in advance for any help.Russ
