Results Page - Pass vs Fail

Hi Everyone - I'm having problems with our LMS correctly capturing complete status...I have v9, publish to SumTotal.
I have custom Pass and Fail result pages in my course.
-On the Pass page there's a score variable from the course assessment and a Finish button. The finish button has variables - Set lesson to complete OnShow; javascript:top.window.close() OnClick.
-On the Fail page there's the score variable from the course assessment; either click to return to beginning of course or click to re-take the course assessment. I don't have anything on the Fail page that reports back to the LMS.
My attempt was to keep the learner looped into the course until they pass the course assessment. For some learners, on the LMS the AU says incomplete but on their transcripts the status says score 100 successful. For others they may receive a Pass page but the LMS still says incomplete for both the AU and on their transcripts. Any idea on what may be going on?
