What publish options are you using when you publish and what are you publishing to (HTML, single file executable, AICC, SCORM)?
If HTML with that many questions you likely will need to publish with Include Title Manager Frame option checked. Try that and let us know how it goes. If that solves your problem you are likely running into the 4K cookie limitation and this should solve it.
