Exit Title action vs. CMI_Core_Exit

Hello,
I'm trying to wrap my head around the CMI_Core_Exit variable in SCORM 2004. I'm having an issue where a learner takes a quiz and submits it and gets a score. They can then see the results in an LMS generated report showing them the questions and their answers. Everything is GREAT at this point. However if they re-take the quiz to get a better score, when they look at the LMS generated report, the questions are in there twice (from both attempts). It's like the LMS is grouping multiple attempts into one and thus giving me a report with duplicate questions.
The vendor is telling me it's an issue with how I built the SCO, which is perplexing because I've been doing this for 7 years and have never seen this issue in any LMS I've ever worked with, but I digress.
So up until this point, when a learner exits a title I've always only used a button with the action "Exit Title/Close Window". I have never used the CMI_Core_Exit variable in any title I've ever built. Should I be? If so, where and what value should I set it?

I am prompting to restore the last viewed location
The course is presented in a modal window (which raises the question of what happens if the learner clicks the X button on the window and not my exit button?)
Does anyone out there have any ideas?
