Bizarre behavior with hyperlinks in V12

Occasionally I've seen hyperlinks go dead. I thought it was because they were in a table so I got rid of the table. But it happened again and it won't let me correct it. One link was dead (address was blank when double-clicked), so I put the address in. Then the 2 links above them stopped working and wouldn't even open the hyperlink dialogue when double-clicked. They were still blue, however. So I deleted them and wrote new text and added new links. However, this is where it gets really weird. The links kept switching to other addresses. I finally When I have the cursor active in the box, the text is there and the links appear to be correct. However, when I click on any other object, the 3 links I corrected change to a single blue text that reads:
HYPERLINK "##Action 49270"
This is also what displays when published to HTML. Plus the original dead link is now dead again.
What is going on here???

I have similar error with my hyperlinks. Worse, when loading to LMS, we get "resource file does not exist error". Very weird, we published in V11 with no such hassles. Please HELP!
We've seen this happen also, with some links being lost when we make revisions to modules, even in Lectora 11. Same behaviour, where when the link is 'lost' the action just shows as HYPERLINK “##Action somenumberhere″
