Test Results Error

I have two identical Lectora tests - same exact setups - one is for our ADL1 and the other ADL2. ADL1 works perfectly. When I run in debug mode I can get the results to show as 100%, 90%, 70%, etc. for exactly how I answer. When I run the ADL2 test in debug, I only get either a 0% (if I don't answer anything) or a 100% (I get a 100% just for all the questions being answered?). When I look at the results, it clearly states 100% but lists my incorrect answers. I have compared action for action between the two ADLs and cannot find a difference. Thoughts?
