Multiple Question, Multiple Answer Doesn't work

I created a SIMPLE knowledge check. I inserted a question - multiple choice multi answer - on a page. There are 12 options, 9 of which are correct. For each of the 12 options, I've added correct text and included an image with the choice. If the user selects the 9 correct options, the "next" button appears, allowing the user to move forward. There is nothing fancy here. I don't need the question "scored" because it's not part of a quiz or assessment.I programmed the Submit button to show a "Sorry, that's incorrect" graphic when the user doesn't select the 9 correct answers. The "Sorry" graphic appears no matter if I answer the question correctly. I have published this to HTML. Please help...this should be SIMPLE!
