Form submitting from SCORM 2004 in SLK

I'm using a form at the end of our courses to capture some feedback. The courses are published as SCORM 2004 and deployed on our LMS which is SLK. The form submits the data to a CGI script. This works just fine when the course is published as an executable (or even just running as preview in Lectora) but when publishing to SCORM and launching from our LMS, it gives an error when the 'submit' button is clicked, and the data doesn't go through, nor does the course close out.Help.
