Error Reading Single Field Image Stub Program

I took the patch (12.1.1?) that was pushed to Lectora Inspire users the other day. I am adjusting a template that was created in Inspire v11, but I will begin using it in v12....so I'm making sure everything works. When I published Offline to an executable, I received the following error (see attached image).
I have never seen this before....anyone have any ideas? I click on the red error message, but it doesn't take me anywhere...so I have no idea from where this error generated.
Help!
