Why is lectora asking for 3D icons?

Aselman is correct that the thid number represents the number of colors. It sounds to me that the file you are telling Lectora to use only has one of the three icons in it - when Lectora is expecting an icon file which contains all three versions of the icon - 16x16x16, 32x32x16, and 32x32x256 all packed in one .ico file.
