Test results showing correct questions when "only show incorrect" selected

When I take a test from my CourseMill LMS the test results page shows 17 of the 45 questions even when they are correct. The score comes up correctly.
They do not display when I take the test from Lectora in the preview mode. I did change the color of the incorrect answers to Red from the results page.
The questions are coming up and saying "your answer..." and the "correct answer" as the same.
What's the deal?

Do you have any " or ' in the question text or in the text of the correct answers? I could solve a similar problem by removing these symbols from the question.
