ReviewLink - Comments go in NULL location

I've had several problems over the last few months with ReviewLink: New Comments do not associate with the page the user is commenting on. They go to a Location of NULL (see attached example) or Untitled Slide. Then the comment cannot be Viewed in Content because there is no page associated with it. I have worked with Support on this issue and we cannot find a reason although it has happened for several Lectora developers and for several clients making comments - all using various platforms. So I am asking others to please report if or when they have experienced this. Thanks!
