"You must answer all of the questions to continue" even though question is answered

Has anyone had this issue?
Sporadically, users will start a quiz. When they get to the first question, they answer it and then click submit. They will then get an error message that says "You must answer all of the questions to continue." I've physically watched them answer it and still get the error message. If they exit the eLearning and go back in, it will then work fine. It seems to be sporadic, only see it on IE7 users and only started with courses published in Lectora 11.3.1
Any thoughts?

Now seeing this issue with sporadic IE 8 and Chrome users. Putting in compatibility mode helps, but it shouldn't be needed. I attached screen shot.
