page not displayed in test section

I appologize in advance if the answer could have been searched. I did try, and nothing jumped out as a solution.

I have created my first 'module'. I was able to publish it with, no errors.

However, I found that my assessment page is not displaying. Attached are screen shots of the title and preview window. I 'think' the menu hierchy is correct? The module work fine, pages advance normal and display OK. But when I get to the first page in the assessment, for some reason it does not display anything but the design template. It will not show the page from the title, nor any of the buttons I've added.

Ideas?
