Using a Survey

I am creating a course in which the user can indicated a like or dislike for information on each page of the course, which consists of several lessons and a final test. To get a feel for this, I created a survey chapter with several true/false survey questions on their own page and indicated that when completed to go to a conclusion page. On the conclusion page, when I add the Submit/Process Test/Survey action, the name of the survey chapter does not appear. Can anyone point me in the right direction in order to use the Survey correctly in this scenario????
