Randomized Test

I have a randomized test that is not working. After failing the test, a page displays with two buttons, one to exit the course, one to retake the test. The button to retake the test has an action to reset test/survey and on click displays goes to the test. On subsequest attempts, not all of the questions display, and the score is not being sent to the LMSI have a randomized test that is not working.
