I don't use tests at all, always just stand-alone questions (since I only use them for non-graded practice activities). I also don't use Lectora's built-in feedback functionality -- I use my own actions (attached to a "Check Me" button that I create) that populate a text box on the page with the relevant message, based on the response the learner makes to the question.
However, if you don't want to use any kind of buttons to make the the feedback appear (you just want it to appear as soon as the learner clicks on their selection), you can use form field checkboxes/radio buttons and just use the On Change action each checkbox/radio button to show a hidden text box with the feedback for that response.
