linking several question to one submit

I wish to create a scenario where by 4 multiple choice questions appear on the same page, the user makes a selection for each and then presses the submit button. They receive custom feedback based on all four being correct or incorrect. Basically 4 questions working as one.I have this much working. Now, I need to ensure that the LMS will track a correct response as 1 point and not 4. How do I link all four correct answers to meaning 1 correct response to the LMS?thanksT
