Do have control over the LMS ? If so, create random questions with answers like ben explained in his post, and make sure that one user (check lms for info) gets access to one course only. Once started, he can not do it again on another time. If you have no control over the LMS, than I am afraid it will be a matter of trust towards your clients.You could try to personalize courses as well. Our LMS does this by printing authentical certificates at our place only, so no matter how many people log in 1 man's account, only one certificate will be printed, and only one will be graduated. P.S. : My company builds courses mainly for workers in the construction section (building roads / buildings and such) in safety. This means that they participate in the courses, get a certain percentage when they make an exam, and based on those results, we send them their certificate. I do not know if your field of work or clients are comparable to this.
