Reviewlink - Reviewer ability to click OK or NOT OK

Has anyone figured out how to activate these buttons?
Is there a magical order of actions/tasks to have the buttons turn from inactive status to active status?
I've tried Trivantis support but thus far, their proposed solutions haven't worked. I assume they are still working on it.
Here are some of their ideas:
They have to go in and refresh their screen AFTER you have marked it as Fixed.
The content must be in a "Fixed" state by the publisher before a reviewer can mark as "OK" or "Not OK".
1. We even tried having the reviewer log out, clear their history, close their browser, re-open, log in again and no go.
2. We tried posting a new comment, then having the publisher click FIX and then have the reviewer try, even if had them try again after repeating 1 above.
3. We tried republishing and using the change all comments to FIXED, still no go.
It could possibly be a bug they still need to fix. In the meantime, I just wanted to see if anyone else has had luck.
