Windows Error Message - persistent data

Through trial and error, I have solved my own particular problem with this persistent data error.The last thing in my course is a post-course survey that contains a series of likert scale questions. As soon as I reached the first of these questions, the error occurred.I changed the questions to multiple choice and eliminated the error. Apparently there is some sort of problem with the likert question type, at least in this case.
