Using Forms to gather learner feedback

I have a course that I want to provide the student the opportunity to email feedback from any screen. However, If I use a button and email, it won't capture which lesson/page number the student was on. This is important so I can review the content.I thought of using a form, but 1) don't know how to use the forms in Lectora yet, and 2) don't know if a form will do what I'm trying. Any suggestions would be appreciated.
