I have seen some similar issues earlier. If I remember correctly they may be related to the IE 4KB cookie size limit. Have you tried with TitleManager frame enabled as well? I think this will bypass the issue by dynamically writing (using DHTML) to the hidden frame instead of storing everything in cookies.I believe this corrected the problems we experienced on large tests/ long test questions/answers.
