I had the same problem and discovered that if the text of any of my answers had an apostrophe in them, the question would be marked as incorrect (even if the correct answer was chosen). I fixed the issue by taking the apostrophes out of my answer text, unchecking the "initially visible" box for the answer text (in properties), and creating a regular text box that contained the answer text with the apostrophes.That way the question gets graded correctly and the users still see the apostrophes.
