I had a similar problem. Open each of your test questions and make sure there are no strange characters in them (like a square -->  instead of an apostrophe). With ours we had copied and pasted in the text for the questions and it resulted in some strange characters which caused Lectora to skip recording the question - even though it showed up as correct.
