I am not sure I understood your post. If you want to have items in a test constructed without using formalquestions, here is what I do. I take a simple question like a Fill in the blank and hide it (make it not visible). Then, when the user clicks on the correct thing, I set the question variable to the correct answer. If not, I leave it blank. They never see the fill in the blank question but Lectora scores it like any other question.
The other choice which is much harder, is to compute the test score manually by counting all the correct answers and computing the test score yourself and then set the AICC_Lesson_Score variable yourself.
