Reset action seemingly causing problems

So we have 7 chapters, and each have a chapter test at the end. Each test is randomized with 10 questions. We put the reset test/survey action on the Did Not Pass results page we created, so that when the user goes back to retake it it will be reset. Chapter1 is working fine, all the other chapter tests are acting really funky. There will be more than 10 questions, it will repeat questions, and will go on forever. Would this have anything to do with the reset actions?
