"Multiple Tries" Question

Hello,

I've checked the blogs, forums and Google for the most complete answer to the "Multiple Tries" issue and I have not found it.

A brief rundown of the scenario is: 1) user takes test and lands on multiple choice question; 2) user clicks an anwer; 3) test displays "Incorrect, please try again" 4) user selects another answer that is incorrect; 5) test displays "Incorrect, the correct answer is..."

If the user selects the correct answer, the test displays "Correct!" with a short justification.

What are the step-by-steps to accomplish the above?

I found one post that included a zip file, but it was old and the zip file is no longer available.
