Publishing to HTML Loses Formatting on Assessment

When I publish to HTML, everything in the course runs fine until I get to the Assessment. At that point, only the text shows up (on my assessment intro page that I created) with no formatting, no background image, no other images on the page, and no navigation arrows. It's the same for every page from that point forward (all the questions and results pages). If I publish to CD, everything is fine. If I look at the page in preview mode, all if fine. If I preview it with a browser, all is gone. This just started happening. Last week when I published this program, everything worked perfectly. The only thing I did today was replace two flash movies and one external pdf file. I did nothing to the Assessment.
Ben Pttman suggested there could be an error on inherited actions in the assessment, but I can't find it. I've uploaded the file for Ben to look at.
