You could have a form with the page identification in it automatically and have it sent to you using the Submit button on the form. Have a button on all pages (at the title level) that saves the current title, chapter, section, and page names, and maybe even the page number, time, and date. Then it goes to a popup page which now puts those saved variables into hidden entry boxes on the form. There is one big box on the form that allows the learner to enter the problem and when they click the Submit button the form is emailed to you.
