I/O Error, cannot send results: Connection reset

hi all,
I have created Form for learner feedback using lectora 2004.
If i click submit button iin lectora mode, it was working.
Ones i published this into HTML version then i clicked on the submit button its showing error message " I/O Error, cannot send results: Connection reset".
Please help for this issue..
