Inconsistent Test Reporting. HELP PLEASE

We are having serious issues with inconsistencies in test reporting. We have Coursemill 4.0 and are creating our tests in Lectora v9.2.


We have been creating these tests from a template for the past 2 years and only just now have started to run into this issue.


Sometimes all student test interactions will be recorded successfully. Most times not.


Sometimes a student’s “status” completion report shows “passed” with score recorded. Sometimes it will show “incomplete” with a “0” score. Even if they have passed the test.


The only way we have been able work around this issue to prove students have completed and passed tests is to implement a certificate page they must print that displays both their test score and a dynamically generated a confirmation code generated from their login ID.


I’ve read the posts regarding eliminating quotes (‘) and (“) in test questions that affect reporting. I am currently looking into that, but would that explain the intermittent success/fail of interaction reporting?


Our tests are used for highly critical functions that are regulated. We absolutely must be able to capture all scores, scores, and test answers 100% of the time.


Is anyone else having this problem? Any help would be greatly appreciated.
