Thanks Ben, that did the trick! It was probably because I initially created the CSV in Excel, and it didn't fill in commas to indicate the empty cells.
Something else that is small, but annoying: the file format info says to enclose all text fields with quotes, however, these quotes are imported as part of the question text. If you do not use quotes in the CSV, it still imports the text fine.
