I didn't understand the multiple layer concept at first and wanted to add this for future searchers...The .ico file must include a 16X16 16 color, a 32x32 16 color, and a 32x32 256 color layer. All three must be there for Lectora to recognize the .ico file.You can add all three and save as an .ico with the program below.
