How can I store my survey results in Google Drive?

I already found the steps for using a form template in Google Drive to store the survey results in a spreadsheet.
But when I use the link https://drive.google.com/templates? to search for the Lectora template, I can’t find any template. I can only see my own documents.. does anyone know where I can find the right template to store my survey results in a Google Drive spreadsheet?

Thank you very much for your answer. I can open the hyperlink and can also view the form, but I don't know how to make my own document of this template and submit it to Google Docs...

I created a copy of the template with a maximum of 50 questions in it. I thought I didn't had to edit the questions in the template, but I can't see the answers on the questions in the submission form after I submitted the survey. I got the notification for one new reaction in the form, but each question said 'now new reaction for this question'. I used the likert scale question in Lectora.
Do I have to add this question type (likert scale) to the template in Google Drive?
