Help: Index for Randomized Test

Thank you for all of your ideas and words of encouragement. I now have the index page as described above and have it opening in a pop up window when the user clicks 'index'.I still have two issues though and I thought I would see if I could get some input. 1) When adding a hyperlink to the question, it won't let me hyperlink to any page within the test (I'm guessing this is because it's a randomized test).2) We are wanting the index page to be the last page seen before submission. (user can click next, next, next... and once the last question has been asked it goes to this page allowing the user to review before submission).
