Form Error in Executable

I have created a sample login program that retrieves MS-Access data and inserts it into a Lectora program. It works fine when I publish to HTML, but I get the following errors when I run the stand-alone executable.

[ATTACH=CONFIG]120[/ATTACH][ATTACH=CONFIG]121[/ATTACH]

form_FN.asp is one of the scripts that I am running to retrieve data from the database. I get the same error message for all scripts.

Any thoughts?
