Customizing Test Questions

Once you create the test, you can format the text boxes however you'd like and position them wherever you'd like on the page.
As for your second question... You certainly can use question objects without embedding them in a test. And, in the Question Wizard, you can use images, instead of (or in addition to) text, but selecting the "Show image with choice" checkbox and providing the appropriate image file.
