Custom test questions?

Is it possible to create a test question that uses a simulation - say, for example, click on the correct button to open blablabla. The user will then click the correct button and the test question is marked as correct. If they click outside of the button, the question is marked as incorrect. I tried to use hotspot, but this didn't work for me. Any suggestions? Thanks in advance!
