Additional Question Being Added and Included in Score of Test

Yes, the question is in the course. One thing I thought it might be was that at the end of the course/test, a have a "submit results" page that defaults to inhereting all variables, including the variable "Question Count." However, all of our tests are set up that way (there are hundreds of them) but this issue only occurs rarely and I have been unable to recreate the problem...Yes, the question is in the course.
