Additional Question Being Added and Included in Score of Test

We are having intermittent errors with users where after finishing a test, an additional question is being added and included in the score. This is not happening to all users in a classroom and these tests are being administered through Internet Explorer. Just curious if anyone out there has seen this type of issue before and can lend some input.
