114: Transaction error - adding pages

Sighle (course owner)Fred (developer)1. Sighle checked out title level2. Fred checked out Page 1 level and got error - "Check Out Failed: Would you like to update the checkout status of the title? OK, Cancel."3. While Fred had message up, Sighle was adding pages -page 2 and page 3 appeared and then the error "114: Transaction error." appeared (see details after this list of steps)4. fred clicked OK and sighle checked in the changes for the title level5. fred did a refresh and does NOT see the new pages6. sighle left title and came back in - Page 2 not there; page 3 is there7.
